Consent for School Health-Related Services - REQUIRED

In accordance with the law, the District must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. This law will go into effect September 1, 2025; please complete the attached consent form by this date. If we do not receive this form back, school-based health-related services that are listed on the form will not be provided. Please reach out if you have any questions or concerns.
